Medical Cannabis Certification
Dr. Mulkey is a licensed certifying physician under Alabama's medical cannabis program and can evaluate and certify qualifying patients for medical cannabis treatment.

We recognize that for some patients, conventional treatments alone have not provided sufficient relief. As a licensed certifying physician under Alabama's Darren Wesley "Ato" Hall Compassion Act, Dr. Mulkey can evaluate patients and, where appropriate, certify them for medical cannabis treatment through the Alabama Medical Cannabis Commission.
We approach this service the way we approach all care at Gardendale Pain & Wellness — with faith, honesty, and the patient's whole person in mind. We believe in approaching all medical treatments with wisdom and moderation. We recommend starting with the lowest dose, and then titrating up as needed to reach your best possible quality of life and functional status.

Who May Qualify
Alabama law permits medical cannabis certification for a range of conditions including chronic or intractable pain, PTSD, anxiety, depression, epilepsy, Parkinson's disease, multiple sclerosis, spinal cord injury, ALS, Crohn's disease, sickle cell anemia, Tourette's syndrome, HIV/AIDS-related symptoms, cancer-related symptoms, and terminal illness among others. This is not a complete list. A full evaluation is required to determine eligibility.

Existing Patient Evaluation and Certification: $200
Quarterly Follow-Up Visit: $100
These are cash-pay services and are generally not covered by insurance. Products are obtained through Alabama-licensed dispensaries only.
