Do I need a referral to be seen?

For chronic pain management, yes, you need a referral. For medical cannabis certification evaluations, no, you do not need a referral. Please call our office at (205) 608-8199 and our team will walk you through exactly what you need to get started.

How do I schedule an appointment?

Call us at (205) 608-8199. Our team will help you find a time that works for your schedule.

What should I bring to my first appointment?

Please bring a photo ID, your insurance card, a list of your current medications, and any relevant medical records, imaging, or prior treatment history related to your pain.

What can I expect at my first visit?

Your first appointment is a thorough consultation with Dr. Mulkey. He will review your history, listen to your concerns, and work with you to build a personalized care plan.

Insurance and Billing

What insurance do you accept?

We accept most major insurance plans. Please call our office at (205) 608-8199 to verify your specific coverage before your visit.

Are medical cannabis evaluations covered by insurance?

No, this is a cash pay only service. It is $300 for the initial certification visit, and then $100 quarterly for ongoing therapy management.

Are chronic care management services covered?

Chronic Care Management and Remote Physiologic Monitoring are covered benefits under Medicare Part B and many private insurance plans. Co-pays, coinsurance, and deductibles may apply.

Your Care

What conditions does Dr. Mulkey treat?

Dr. Mulkey treats a wide range of chronic pain conditions including back and spine pain, neck pain, joint pain, neuropathic pain, and complex chronic pain that has not responded to other treatments.

What procedures are available?

We offer epidural steroid injections, joint injections and nerve blocks, radiofrequency ablation, spinal procedures including spinal cord stimulation and kyphoplasty, topical pain compounding, oral ketamine therapy, medical cannabis certification, and chronic care management with remote monitoring.

Will I be required to take a drug test?

As part of our chronic pain management program, periodic urine drug screening is required for all patients receiving controlled substance prescriptions. This is a standard patient-safety measure used to help ensure compliance with treatment, identify potentially dangerous medication interactions, and meet regulatory and professional standards for the safe prescribing of controlled substances.

Is everything I discuss with Dr. Mulkey kept confidential?

Yes. All patient information is fully protected under HIPAA. Your records are never shared without your written consent.

Is Dr. Mulkey licensed to certify patients for medical cannabis?

Yes. Dr. Mulkey is a licensed certifying physician under Alabama's Darren Wesley "Ato" Hall Compassion Act.
